Orchestra Families Families, please fill out this form so that we know if you need a loaner instrument and microphone or if you will be renting/purchasing your own.
Band Families, you only need to fill out this form if you will be using a loaner instrument or microphone. If you will be renting your own instrument and purchasing your own microphone, you can ignore this form.
Instrument and equipment pickup is Wednesday, September 28 at WMS, during two time slots: 10:00am -2:00pm or 4:30pm -7:00pm.
